The Liability Waiver and Health Questionnaire and Disclosure Forms must be completed 24 hours before your first session.

Please click on the Download Forms button and download the Waiver and Questionnaire Forms. 

Once you have completed the forms please upload the documents by clicking on the Upload Forms button.
